Hiring Documents
The following Student Employment hiring documents, in addition to the Student Employment Referral, should be printed and completed with the student at the hiring department once employment has been secured.

Domestic students
- Hiring Checklist
- Job Classifications List
- Employment Eligibility Verification (I-9) Form
- Federal/State Withholding Allowance Certificate (W-4)
- Employment Data Form
- Authorization for Electronic Direct Deposit (EDD) of Student Pay
- Ethics Act Training
- Illinois Mandated Reporter Acknowledgement Form
- Statement of Purpose for Collection of Social Security Numbers - Hiring departments provide to student employees (retained at hiring department).
- (Optional) A-21 Distribution Form - Hiring departments may use this form (not a mandatory hiring document).
International students

International students will need to obtain a visa status verification form from the Center for International Education (CIE), and proof of application for an SSN from the Social Security office, PRIOR to turning in hiring documents to our office.
